I use a Nikon d-60 with a HB-33 lens hood on the 18-55 lens and a SB-800 speed light off camera. The key is to use a frame (ie. Stroboframe) to keep the FLASH CENTERED OVER THE LENS. Also keeping your subject a few feet from any walls will pretty much eliminate your shadow problems. A bonus is this setup also eliminates red-eye.
