What would cause a cat's stool to harden so bad they nearly die?

My friends pure bred cats have been in and out of the vet losing body temp, not defecating and not eating. The vet did surgery and the stool was so hard it was scarring the bowel. After investigating the stool, the vet can't find anything wrong-no foreign particles, no parasites, nothing!
