Do you think some not forgive and give a chance because they feel they lose self-respect?

I don't think it is self-respect they think they will lose in the examples you gave , like taking things lightly, expecting that person to revolve around you, smothering them, & lying to them about something big... Well they do not accept the apology because they feel used. And refuse to be put in that situation by this person again. Not only do they feel used , they are hurt because this person lied about something important , & The trust is gone. No one likes being treated the way you described. & are refusing to be put in that situation by this person again. Someday in time they may forgive to an extent , but it will always be in their mind, & trusting this person again or even wanting to be a around them won't ever be a priority So Sometimes you got to let it go& know you apologized, but the damage is done, & can't be fixed with mere words*
